Why Your TRT Is Failing - Even With PERFECT Test Levels

We’ve all seen it happen on the online forums. A guy proudly posts his latest lab results, bragging about a total testosterone level of 1100 or 1200 ng/dL like he just unlocked a high score in a video game.

But here is the reality check that nobody in the fitness community wants to tell you: You can have mathematically "perfect" numbers on your lab work and still feel like absolute garbage.

Conversely, your numbers might look completely average on paper, yet you feel like a million bucks. If you’ve been chasing a specific number on a page while ignoring how you actually feel in real life, you’ve fallen into what I call the Bloodwork Illusion.

Let’s break down the hidden trap ruining your TRT results and look at exactly how to fix it.

Total vs. Free: The Delivery Truck Analogy

When your lab results hit your inbox, your eyes naturally jump straight to that Total Testosterone number. We all want to see a big, impressive stat there. The truth? Total testosterone is mostly a vanity metric. It doesn't tell you the real story.

To understand why, think of your total testosterone like a fleet of delivery trucks loaded with packages (let’s say they’re from Amazon cause they probably are).

  • The Problem: It looks fantastic on paper to have a massive fleet of trucks. But if those trucks are permanently locked behind the gates of a strict gated community, they can't actually deliver those packages to your house. They are completely useless to you.

  • The Reality: In your body, that locked gate is a protein called SHBG (Sex Hormone-Binding Globulin). It acts like a giant molecular sponge, binding tightly to your testosterone and locking it down.

The only testosterone that actually does the heavy lifting, the stuff that builds muscle, fires up your energy, and drives your libido, is your Free Testosterone. That is the unbound hormone that is actually free to leave the bloodstream and enter your cells.

This is exactly why a guy can have a total testosterone level of 800 ng/dL but feel completely exhausted, moody, and weak. If his SHBG is through the roof, his free testosterone is dangerously low.

3 Real-World Ways to Unlock Trapped Testosterone

If your free T is low, your automatic reflex shouldn't be to just demand a higher TRT dose from your doctor. Pumping more testosterone into a body that has sky-high SHBG is just going to create a massive, complicated hormonal mess.

Instead, you need to lower the "sponge effect." Here are three down-to-earth, real-world ways to free up your trapped testosterone:

1. Fix Your Diet (Stop Over-Cutting)

Extreme dieting, crashing your calories, or severe, long-term restriction of carbohydrates will drive your SHBG straight up. When SHBG spikes, it freezes your free testosterone. Your body requires adequate calories and clean, healthy carbs to keep those binding proteins in check. (Note: If you follow a strict carnivore diet, your body may adapt over time, but generally speaking, chronically low carb intake can become an issue here).

2. Add Boron to Your Supplement Stack

One of the cheapest, simplest, and most effective tools in your arsenal is the mineral Boron. Taking 6 to 10 mg of Boron per day has been shown in clinical studies to significantly drop SHBG levels. I regularly talk to guys who are completely symptom-free, full of energy, and dialed in with an average total testosterone level of 650 ng/dL or even lower. Why? Because their free testosterone is optimized and their lifestyle is locked in.

Don't try to fix what isn't broken just to satisfy a number on a spreadsheet.

TRT is designed to fix a medical deficiency; it cannot fix a broken lifestyle. If you are only sleeping four hours a night and eating junk food, a perfect 1200 ng/dL total testosterone number isn't going to save you.

10 Weeks on TRT - Get Fit Over 40

My last update was almost 5-6 weeks ago so I guess I better get a TRT progress video out for everyone. To be honest I noticed the most changes after around the first month and then things slowed down and progress is still happening but much slower now. I am only on 100mg of Testosterone per week with the goal to be i the middle natural Testosterone ranges so I don't expect to get super jacked on this fairly low TRT dose, the goal is really to feel better and at least have a chance in the gym getting stronger and being able to recover between workouts.

When I started my TRT I was around 185 pounds and fairly lean I would say, right now I am around 192-194 depending on the day and I am still relatively lean but I do want to get closer to 190 with perhaps a bit more muscle but I would like to get rid of some of the hard to lose fluff in the mid section.

I feel pretty good overall, even my shoulders are coming around and my overall energy and mood are good, sex drive I would say is back to about the way it was but not like when I was 20 ;)

TRT 5 Weeks In - How Do I Feel

A week back I made more of a physique update video after being on TRT (Testosterone Replacement Therapy) for a month now (Dose of 100mg per week). I didn't really go over how I am feeling now where I talk about stuff like mood, energy, motivation, sleep, mental cognition, sex drive. These factors are just as important or maybe even more important than what the TRT does for me physically and for sure need to be talked about and that's just what I do in this video.

TRT One Month In

A month into my TRT (Testosterone Replacement Therapy) and there have been some changes for sure! During this time I did do a 2 week mini cut in preparation for my Cruise trip, damage control if you will. Of course the cruise itself was a lot eating and so this too is a factor but I am going to say that the mini cut did offset some of the eating for the most part but not all of it as I feel I am up a little bit up in body fat from pre TRT.

I started out at 185 pounds, two weeks in I was 190 pounds but again keep in mind I was mini cutting all that time. I then went on my Cruise and of course gained some weight, pretty sure I hit 202 on the ship but bounced back down to around 195-197 after a few days back eating a bit better.

So now 1 month into my 100mg per week TRT and I am sitting right around 196-198 for sure holding more muscle than before TRT and I do feel stronger in the gym. I want to trim off some of the fat and get down to around the 190 mark as my goal now is not so much size but rather health and if I can be a lean 190 pounds, I know for my frame that will look good and not be to strenuous on my body or require a lot of work to hold on to.

So for now I will stay the program and see where it takes me, I am happy so far with how I feel and the results I am getting in the gym and how I am looking. I will have an upcoming appointment with my TRT doctor to make sure my Testosterone is hitting the mark he has set for me of around 22 nmol/L. I may need to make an adjustment afterwards but for now things are feeling a lot better.

Post Cruise Physique Update

So I am not going to lie, I for sure gained some weight on my cruise as was expected! At one point on the cruise ship I was up over 10 pounds hitting 202 pounds, I am now back down to around 195 pounds which is still 5 pounds over my weight before the Cruise.

I am actually pretty happy with this, as I feel I lost close to 5 pounds of fat leading up to the cruise and so my weight now is more of a refection of what the TRT is really doing as my pre TRT weight was around 185 pounds. Not all of the 10 pounds is muscle, but some for sure is, the rest will be water weight and maybe just a bit of fat but I am feeling stronger and I feel the work I am doing in the gym is now paying off more.

I do want to get down closer to 190 pounds and be a bit leaner, I am fairly happy with my muscle mass now but of course I don't mind gaining a bit more muscle as long as I keep my weight down around that 190 pound range, so being leaner is for sure a priority...

I did try a different camera for this video as I was not 100% happy with the Hovercam X1 video as it seemed to be a bit grainy in the indoor lighting I was using. I should of stuck with the Hovercam X1 for continuity reasons as the Insta360 I used with the 1 inch lens I feel softened the look of the video and blew out the contrast a lot which does not make for a good comparison.

I will probably switch back to the Hovercam X1 for my 1 month post TRT Update Video I plan to shoot tomorrow, so hopefully I can properly represent the changes that have been taking place over the past 4 weeks.
